Game keeps Crashing/Freezing when ever i drive.

So, it does this with Custom shaders patch, it sometimes does it without it too, I am clueless why, it doesn't log or anything my screen just goes black, I can only close the game with task manager, I've been trying to fix this for ages help ;-;

Sometimes it doesn't even load, it just crashes on the loading screen.
